QuickDate v2.3 IOS v2 Written by: Dr. Jane Smith Published in: Journal of Rheumatology Date: March Abstract Rheumatoid arthritis (RA) is a chronic autoimmune disease characterized by inflammation of the synovial joints, leading to pain, swelling, and potential joint destruction. Early diagnosis and treatment are crucial in managing the disease and preventing joint damage. This study aimed to evaluate the effectiveness of a new biomarker, anti-citrullinated protein antibody (ACPA), in the early diagnosis of RA Methods A total of 150 patients with suspected RA were enrolled in this study. Patients were divided into two groups: those with positive ACPA (n=90) and those with negative ACPA (n=60). Clinical and laboratory parameters, including erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), and rheumatoid factor (RF), were assessed. Patients were followed up for 2 years to evaluate disease progression and response to treatment Results The ACPA-positive group had significantly higher ESR, CRP, and RF levels compared to the ACPA-negative group (p<0.001). The ACPA-positive group showed a higher prevalence of joint involvement (p<0.001) and radiographic progression (p<0.001). The ACPA-positive group also demonstrated a higher response rate to disease-modifying antirheumatic drugs (DMARDs) (p=0.001) Conclusion The presence of ACPA in patients with suspected RA is a valuable biomarker for early diagnosis and treatment. ACPA-positive patients have a more aggressive disease course and respond better to DMARDs. Incorporating ACPA testing in the diagnostic workup of suspected RA may improve patient outcomes and reduce joint damage Keywords: Rheumatoid arthritis, ACPA, early diagnosis, biomarker, disease progression, DMARDs, synovial joints
